Benefits through Pacific Blue Cross

FNHA Benefits can be direct
billed through Pacific Blue Cross using your status card number. Eye exam
coverage is $100 and prescription eyewear coverage is $275. Coverage renews
every 2 years for adults and every year for children under 19 years of age. You
can find out your insurance coverage by downloading the Pacific Blue Cross app
or calling 1-877-722-2583.

Children 0-5 yrs: $25

Children 6-10: $50

Children 11-18 years of age: $99

Children contact lens exam add-on: $50 (For current CL wearers) $40 toward first order of contacts.

 

Adult eye exams: $144

Contact lens exam add-on: $50 (For current CL wearers) $40 towards first contact lens order 

Senior eye exams: $144
